Before your first appointment, you will need to fill out an intake form, which will be available to you in advance.

All fees are due in full at the time of the session. Reduced fees for services are available on a limited basis.

Payment can be made by cash, check, Quickpay or Zelle, PayPal, Square (2.6% additional charge for Square).

If you need to cancel, please do so within 24 hours of your scheduled appointment. A full fee of $165 will be charged for any cancellations less than 24 hours before appointment time.
